The Contact Lens Fitting (CLF) is a specific exam for the proper fitting of contact lenses. Superior Vision offers a stand­ alone CLF benefit that enables members to maximize the value of their contact lens allowance. Two Types of Fittings. Standard Contact Lens Fitting. Our Superior Vision plan is designed for you to choose a provider that fits you and your family’s vision needs at an affordable price. When you use a Superior Vision provider, you are responsible for a small copayment at the time of service. Your provider will file a claim for you and be reimbursed directly from Superior Vision.

If you utilize a non-network provider, you pay all expenses at the time of service and must submit a claim for reimbursement up to the allowance shown in the Vision Plan Highlights chart below. Whichever provider you choose, Own It!

Remember to ask your Superior Vision provider about special discounts for additional pairs of glasses, special lens options and other vision services including LASIK surgery.

VSP Buy-Up Plan

In addition to the Superior Vision plan, you’ll have another option: the VSP Buy-Up plan. This new options provides a higher level of coverage, so it comes with slightly higher paycheck costs.

The University’s vision plan, administered by Superior Vision, covers eye examinations, prescription eyewear and contact lenses.

You have two options for vision coverage: the Basic Plan and the Enhanced Plan. You have the option to see a provider in the Superior Vision National network or an out-of-network provider; however, you’ll always pay more for out-of-network services.

The table below shows what you will pay for in-network care. Out-of-network care will be reimbursed after you submit a claim, up to plan limits. Copays for out-of-network services will be deducted from your reimbursement.
